12 people are missing after an explosion that hit an optics plant in Sergiyev Posad, a city in Russia’s Moscow region, on Wednesday, according to the country’s Emergency Situations Ministry.

Regional health officials reported that 14 people injured in the blast are still in the hospital and that three of them are in critical condition.

The Russian authorities attributed the explosion to “human error.” At least 60 people were reportedly injured and at least one person died as a result of injuries sustained in the blast. State investigators have opened a criminal case over the alleged violation of safety requirements and arrested the technical director of Pyro-Ross, a pyrotechnics company that they say was leasing the warehouse where the blast occurred.
